Learn the ICD-10-CM Official Guidelines rule by rule, then practice them on short scenarios and note-style cases.
This 25-page printable workbook covers diagnosis coding for coding students and new outpatient coders, built on the FY2027 code set (valid October 1, 2026, to September 30, 2027).
What is inside
- 47 Official Guidelines rules quoted word for word, each with its section number
- Conventions: placeholder X, 7th characters, Excludes1 vs Excludes2, "with", combination codes, "use additional code" notes
- General rules: signs and symptoms, laterality, who may document (BMI)
- Outpatient (Section IV) rules: first-listed code, uncertain diagnoses, chronic conditions
- Diabetes and Z79 drug codes, hypertension with heart and kidney disease, injuries and fracture defaults, poisoning and the Table of Drugs and Chemicals, sequela, history and pregnancy Z codes
- A six-step Index-to-Tabular method and 4 worked examples
- 53 coding drills (write the codes, in order), 12 of them written as short office-note excerpts, and 20 multiple-choice questions
- Answer keys with a short reason and the guideline section or Tabular List note for every item
Scope, so you know what to expect
A focused set of 53 drills and 20 multiple-choice questions on the general and outpatient (office) guidelines only, with no inpatient rules. There is no timed exam; a score log lets you record up to four attempts. To use the 20 questions as a mini mock test, cover the key, allow about one minute per question, then score yourself with the key.
Every answer code was checked by computer against the FY2027 ICD-10-CM code file from CDC's National Center for Health Statistics, and every code title in the answer key is copied from that file.
Diagnosis coding only: no CPT or HCPCS content. A study aid for exam practice, not coding advice for real claims.
How it was made
